Documents Required

• A photocopy of the passport page with the photo of the bearer. (Valid passport).
• Private address.
• Father's full name.
• Mother's full name.
• Highest level of Education.
• Marital statur.
• Place where the birth was registered.
• Arrival and Departure Date.
• Tourist card number.

If divorced—Decree absolute  (translated into Spanish and legalized at the Cuban Consulate).
If widowed—Death Certificate  of former spouse and previous Marriage Certificate.
If your name has been changed  by Deed Poll-- legal proof  (also  translated into Spanish and legalized at the Cuban Consulate).
